You can view your information through their mobile app (<a href=\"https:\/\/apps.apple.com\/us\/app\/mobile-health-consumer\/id554910281\">Apple<\/a>\u00a0or <a href=\"https:\/\/play.google.com\/store\/apps\/details?id=com.mobilehealthconsumer.mhc&amp;hl=en_US\">Google Play<\/a>) or by going to their <a href=\"https:\/\/www.mobilehealthconsumer.com\/web\/pages\/login.html\">website<\/a>. To check your credits, from the home screen, click on Wellness Program and you will then see the requirements you have completed. As a reminder, employees and spouses covered on ACU medical plan must complete the wellness requirements by September 30, 2022, to receive wellness credit towards their 2023 premiums. The two requirements are to complete an annual exam with your doctor and receive a biometric screening or preventative lab work.
