Go to <a href=\"http:\/\/My.BlueprintForWellness.com\" target=\"_blank\">My.BlueprintForWellness.com<\/a><br \/>\n2. Enter Registration Key: <strong>ACU2015<\/strong><br \/>\n3. Enter your Unique ID: First Name, Last Name and first 4 digits of DOB (For example: Willie,Wildcatmmdd)<br \/>\n4. Complete the registration process online as directed.
