To check if you have completed your wellness exam, please see the instructions below.<\/p>\n

If your exam was between 10\/1\/15-12\/31\/15, you will need to login to your\u00a0myCigna account<\/a>. Once you are logged in, you will click on the “My Health” tab at the top of the page. You will click on the blue button in the middle of the page on the left-hand side that says “View My Incentive Details.” All of your claims will be listed here and you are looking for a preventative exam. (see examples below)<\/p>\n

If your exam was 1\/1\/16 or later, you will need to login to your\u00a0GPA account<\/a>. Once you are logged in, you will click on the “Claims” tab at the top of the page. You will want to change the “Show” dropdown the the “Last 12 months” and click “Search.”\u00a0All of your claims will be listed here and you are looking for a preventative exam. (see examples below)<\/div>\n

You are looking for an EOB with something like the following listed:<\/div>\n
“PHYSICAL EXAM”,\u00a0“WELLNESS EXAM”,\u00a0“GYN EXAM”, or\u00a0“ROUTINE PAP”<\/div>\n
