A transition is a passage from one life state, stage, or place to another. Occupational therapy practitioners increasingly serve clients at critical times of transition as people experience planned and unplanned transitions throughout their lifespan. This comprehensive text is the first to offer an occupational therapy approach to clients’ transition needs, from the neonatal intensive care unit and school to ageing and end of life. Aligned with the Occupational Therapy Practice Framework, this work explores the various transitions people experience and the ways in which occupational therapy can facilitate better intervention outcomes as clients face changes and challenges in their lives.
As health care systems are currently undergoing many changes, occupational therapy practitioners need to understand how to communicate, document, and plan within transitional contexts for the best possible client outcomes.
