If you were unable to participate in the Biometric Screenings held on campus in May, you can now participate by using a voucher at Quest Diagnostics between June 1st – June 30th, 2015.
To register for a voucher you need to:
1. Go to My.BlueprintForWellness.com
2. Enter Registration Key: ACU2015
3. Enter your Unique ID: First Name, Last Name and first 4 digits of DOB (For example: Willie,Wildcatmmdd)
4. Complete the registration process online as directed. You will also then be directed to complete your CIGNA Personal Health Questionnaire and schedule your screening.
