You can also schedule your appointment through the Mobile Health Website. To download the Mobile Health Consumer app:

If you cannot attend one of the onsite screenings, whether you are a remote employee or due to a schedule conflict, there is a form on the eHealth website to download to complete your screening with your physician. Follow the steps to schedule a screening to access the form. Please review this document for additional information. If you have any trouble downloading the Mobile Health app or scheduling your screening, please contact HR at humanresources@acu.edu.
